ACC patients.
Your personal information (date of birth, address, occupation) and injury details are used when you lodge a claim. When an ACC claim is lodged, you have given consent for ACC to request and obtain information related to your claim, including treatment notes, referral letters and imaging (in addition to your claim).
Your e-mail address and phone number are used for direct contact about your appointments and other questions.
Your personal information and notes will not be shared with any third party without your consent.
Mailing Lists
All new patients are automatically added to our newsletter mailing list to update you with clinic changes and provide educational material related to your health.
As part of the registration process for our email newsletters and workbooks via our website, we collect personal information. We use that information for a few reasons: to tell you about stuff you’ve asked us to tell you about; to contact you if we need to obtain or provide additional information; to check our records are correct, and to check now and then that you’re happy and satisfied.
